Progress Section
In this article you will learn in detail about the overview progress section and engagement sections. For this progress section we are considering the following program types
Collaborative Care
Reduction Readmission (RRP)
POP Health programs
PHMP
IVRP
SELP/SEPP
SPP
Member progress is considered based on the above program types. For the pop health programs we also display the ROI data if available.
Below you will learn in detail on how the progress displays for each of the program type. On mouse hover the progress section we will display more which navigates to the "Progress Tab"
Note: The progress tab details will be available if the member program is either in active or completed status

Collaborative Care (CCP):
For collaborative care the member progress is based on the PHQ9 and GAD7 assessment scores.
This progress icon is same as what you see on the member list. Based on the combination of PHQ9 first, PHQ9 last, GAD7 first and GAD7 last the progress will be determined.
Below is an example of how the member progress would display if sufficient data is available
Reduction Readmission Program (RRP)
For reduction readmission program type, the progress is based on the recent hospital readmission. If there is a hospital readmission then the progress is considered as negative and arrow points downwards and if there is no readmission then the progress is considered as positive.
Along with the progress details we are also displaying the diversion information from the recently submitted case notes

Population Health Billing Program Types
For the population health programs the progress section includes ROI and Assessments related data. Below you will learn which assessments are considered for each program type within the POP health programs

For all the below program types we have the following information in the overview
Total Cost of Care
Hospital Stays
Emergency Dept. Visits
The data is based on the annualized amount and the progress of increase or decrease will be based on the pre-period and post-period

Below you will learn on each program type for the "Assessment" related data
PHMP
For PHMP program type we are considering the M3 and HRSN assessment data to display in the overview section.
For each of M3 and HRSN the initial and the recent data is considered and the difference in the results is shown as delta.
In case of HRSN as there would be no score available, we consider the number of social needs.
The difference in increase or decrease of the social needs will be displayed as a delta value
PHMP follow up main questions (short form) and answers are also displayed in overview.
IVRP
For IVRP programs we are considering the M3 and HRSN assessment data to display in the overview section.
For each of M3 and HRSN the initial and the recent data is considered and the difference in the results is shown as delta.
In case of HRSN as there would be no score available, we consider the number of social needs.
The difference in increase or decrease of the social needs will be displayed as a delta value
IVRP Follow up V2 main questions (short form) and answers are also displayed.
SPP
For SPP program type members we will have two categories
Over 18 years
For members who are 18 and older for the progress we are considering PHQ9 and M3. Also the SPP follow up question and answers.
For each of PHQ9 and M3 initial and recent data is considered and the difference in both the results is shown as delta above the arrow
2. Below 18 years
For members who are 18 and older for the progress we are considering PHQ9 and PSC-Y. Also the SPP follow up and CSRSS- Triage assessment question and answers
SELP/SEPP
For SELP/SEPP program type we are considering PHQ9 and HRSN assessments. For follow up section question and answers from the SUD-FU and SELP/SPP follow up assessments.
For both the PHQ9 and HRSN the initial and the recent results are displayed and the difference in both the results is shown as a delta value on top of arrow
In case of the follow up for SUD FU, the total number of substance usage that is increased and the total decreased for the recent submission is displayed in overview
Engagements Section
Engagement section will have data when there is at least one case note submitted for the member.
This section will have the last successful and the last attempt engagement details, which will be based on the case note submitted by the care extender.
If the last attempt was not successful but there was a successful engagement earlier then we will have both last successful and last attempt details like below
For last successful engagement if there is an
Summary of engagement logs will also be available in this section.
The logs can be filtered for last 30 days or last 90 days
