SUDP Enrollment Cheat Sheet

  1. Review Previous Case Notes

    HOW: Member List View->Outreach Engagements

  2. Check Eligibility

    HOW: Eligibility Tab (Does not apply for ACLA and Texas Markets)

  3. Call Member- Verify identity and confirm pronouns (confirm full name, phone, mailing address)

    • Inform the member that the call is being recorded. (Does not apply to therapist)

    • Update Phone Number Status with “Correct”

    • Mark phone number as “Primary” to activate consent to text messages.

    HOW: Hover over the phone book and mark correct, for phone, address and email.

  4. Obtain Emergency Contact

    HOW: Contact Information->Member->Emergency Contact

  5. Confirm/ Update Time Zone

    HOW: Click “pencil” (by member name to open Demographics). Confirm the time zone.

  6. Complete assessments: 1.) Enroll ASSIST 2. ) HRSN

    HOW: (Assessment Tab)

    *If the member does not meet criteria after completing the Enroll ASSIST, do not proceed with enrollment unless SUD history can be seen in member utilization SUD Enrollment Eligibility Quick Reference Guide

  7. Publish note BEFORE ACTIVE

    USE: Engagement Type: Enrollment Outreach; Intervention Type: Other: Notes: Enrollment

  8. View Intake Calendar for CTM Availability

  9. Assign Primary CTM BEFORE moving to next workflow status

    How: top right hand corner of mMR Program, change status

  10. Schedule Member for ZOOM Intake

    Create Task: USE Intake Appointment w/ Member Appointment Reminders

  11. Obtain and Document BOTH Verbal & Written Consent

    HOW: Verbal Consent →(Program Tab OR Overview - Blue Program Box) - Add date to Detail & Consent

    HOW: Written Consent → (Document Tab → Select correct program if there are multiple → Send via Docusign Button) → Select Consent & Mode of Delivery (Text or Email)

  12. Move Member to ACTIVE status

    HOW: top right hand corner of mMR Program on Overview page

    NOTE: Send email or text Welcome Packet when prompted

  13. For SEPP Members, Add Pregnancy Information

  14. Send Enrollment Email (to Management, Assigned CE & Program Coordinator)

    • Content: New Intake. Market. Program. CE. Date/Time. mMR number (STANDARD SUBJECT LINE-New Enrollment)

  15. OEC Team Only: Update Chat and Enrollment Log
