Describe how the contact or activity occurred. These capture the format of the interaction rather than the specifics of what was done.
Interventions
Interventions describe what actions you actually took during the engagement. They focus on the specific services, support, or steps provided to address the member’s needs.
Use “other” for enrollment calls and as a current fix to the unsuccessful visit requiring an intervention. Please talk to your manager if you find a need for a new intervention type to be added.
Notes on Interventions
Provide a clear, detailed narrative that explains exactly what you did during the interaction to carry out the selected intervention. Focus on describing the actions, techniques, or tools and supports provided—enough detail that someone reading the note understands how the intervention was delivered.
Not required for unsuccessful contacts.
Intake: “Intake Completed. See intake and assessments for details.”
