Discharge Template-MPMP

The CE discussed discharge planning and aftercare with Member in preparation for discharge from the program. The CE completed the following final assessments with the Member [M3, HRSN Follow Up, MPMP Follow-Up, Mindoula Satisfaction Survey, all others triggered] or The Member declined to complete the follow-up assessments and satisfaction survey. The CE and Member completed the Discharge Summary Report, discussed the aftercare for future use, including medical, behavioral and crisis resources.
