Discharge Template-IVRP

CE discussed discharge planning and aftercare with Member in preparation for discharge from program. CE completed the following final assessments: M3, HRSN Follow Up, IVRP Follow-Up, all others triggered (OR DOCUMENT MEMBER’S DECLINATION), and the Mindoula Satisfaction Survey. CE and Member completed the Discharge Summary Report, discussed the aftercare for future use, including medical, behavioral and crisis resources.
