Discharge Details
If Graduation, select all options that apply:
If Administrative Discharge, select the appropriate details
Post-Discharge Recommendations
Document the Member’s existing community providers including any scheduled preventive appointments in the the appropriate open text boxes
Send the Member’s relevant referrals as a pdf via the document referral via the Document Tab of the Member’s mMR record
Emergency Resources and Numbers
Select all options that are relevant to Member and enter Member’s local urgent care and Emergency Department into open text areas.
Finalize Discharge Summary and Share
Confirm with the Member their preferred method of delivery (email, post mail, etc.) and click “Generate”.
If method selected as “Post Mail” please generate report, download, save as PDF and task the appropriate Program Coordinator to mail to the Member
Once Discharge Summary Report is generated, send it to the Member via Documents Tab in mMR
There will be two versions of the discharge summary. Please send Members the Discharge Summary that does not have “CE_Version” in title.
