The Speciality Population Health Management Intake was designed with both the Member and CTM in mind. The flow of this intake is appropriate for all programs and allows a smooth straightforward user and participant experience.
Navigation to the Intake
In the overview depending on if an intake has been scheduled or not there are two options to initiate. You can either select call or begin zoom when an intake appointment has been previously created. You can also navigate to the task tab and join the zoom call directly from there.
Intake Progress
Once the intake is started, you will have an option to view the progress % and an option to continue from where you left the intake checklist. Once the intake is completed, this section clears from the overview.
Intake Tab
One of the Member tabs is called the “intake tab” and it will open the Intake page. Once the intake is done, if there are any edits needed, the team can come to this page to make those changes.
Intake Navigation
The team has to complete the following sections to complete the intake process. Most of the below sections are mandatory to consider completed. On each section there is an option to go to the previous or the next section. All the sections are available in the menu option on the left side. At any time you can minimize the intake and go into other sections of the Member chart. Most information entered into the intake, connects to other areas of the chart, i.e. demographics, medication, etc.

Consent and About SPHM
The text for verbal consent is a reminder for the Member even if consent was gained at enrollment. Please review, if verbal consent was not gathered, clicking on Member consent will complete this process.
For SUDP, status of the written consent is displayed at the bottom.
Demographics
Once all the required details for demographics are gathered during the intake, this step can be marked as done.
Background/Social
This section has a list of questions the team has to fill in while engaging with the Member. Once all the mandatory questions are filled this can be submitted. Once the details are submitted, the question and answers can be viewed in this page and can be edited later if needed.
Social Needs Assessments
If the HRSN was not completed at enrollment, it can be completed here. If the assessment is marked done, you can navigate to the next tab.
Social Needs / Goals and Referrals
Based on the previous steps, the team will add any goals or referrals as needed. Once added the step can be marked as done.
Utilization
This step asks questions regarding hospital and emergency visits. Once answered and submitted, the step is considered done.
Medication
In this section, the team gathers the information on the current medication member is taking. Once the answers are submitted, this step is considered as done
Physical Health Questionnaire
This questionnaire gathers details on the physical health of the Member. Once all the mandatory questions are answered and submitted. This step is considered done
Physical Needs / Goals and Referrals
Based on the physical health information gathered, this step is to add any goals or referrals if any needed
Behavioral Health Questionnaire
This questionnaire gathers details on the behavioral health of the Member. Once all the mandatory questions are answered and submitted. This step is considered done.
Behavioral Health Assessments
This step has the assessments listed to finish related to behavioral health that are program specific.. Once the mandatory assessments are submitted, this step is considered done
Behavioral Health Needs / Goals and Referrals
This step is to add any goals/referrals if needed based on the previous step details gathered.
Safety/Violence Questionnaire
This step has questionnaire about safety/violence & once the answers are submitted this step is considered done
Safety Plan
Submitting the safety plan completes this step. This is a requirement based on program direction.
Member Summary
Based on the information gathered, member summary is generated:
Continue Case Note
Click on “continue case note” and complete the case note submission. The member summary displays in the "Additional Notes" section at the bottom of the case note. The only narrative needed is the write "Intake Completed, See Assessments and Summary for Details."
Filtering for Intake Completion
You can filter for Members with Intakes completed by filtering for program->intake status
Completed Intake
Once the intake is completed you can navigate to the intake tab in order to view the completed intake at any time. Once the intake is 100% completed, it will disappear from the overview page.
