Task Utilization in mMR

In mMR, tasks are the equivalent of a ‘To Do” list. They serve as a reminder for individual Care Team Members (CTMs) to complete work, they are a communication tool for the entire Care Team, they are a method of organization for the Member chart, and they ensure a Member or required process does not get left undone. Utilizing Tasks is not optional, but a required workflow.

Task Creation

  • Open Member chart and choose “Tasks” on the left hand side

  • Click on ‘Add Task” - blue button in upper right corner of chart

  • Fill in from drop down:

    • Task Type

    • Care Extender

  • Task Description

    • Describe what you need in this box to know exactly what is happening or needs to happen

  • Enter in End/Due Date (start date is an optional field)

  • Select Save

Task Types

mMR provides multiple task types. There are many types of tasks that you may use and can be standardized according to your own workflow. The below tasks are the most common across programs:

Assessment

Use in regard to initiating, or updating assessments. Many will be automatically created by mMR.

Follow-Up Appt

Use for any appointments other than intake.

Intake Appt

Use for intake appointments.

Documentation

Use for all internal communication with the team.

Internal

Use for all requests to Program Coordinators

Follow Up

Use for follow up outreach.

Follow-Up: If a Member is disengaged, task weekly and continue to reschedule until contact is made, then utilize appointment tasks. *Do not use Follow-Up Appt unless the Member made the appointment.

Peer

Follow-Up: If a Member is disengaged, task weekly and continue to reschedule (2 times a week) until contact is made, then utilize appointment tasks. *DO not use Follow-Up Appt unless the Member made the appointment.

Therapist

Documentation: Set task reminders for Members at 90 days no contact. Utilize steps of discharge protocol within the task.

Peer

Documentation: Goals and Referrals (Update Every 90 Days)

Click on Tasks icon on left side of screen

Filter Assigned to “your name.”

Important Information Regarding Tasks

  • When a Member appointment is created in mMR, a task is created for participating CTMs.

    • Member confirmed appointments are visible as a green check mark in the CTM task list for the day

    • If Member declines session/appt will show as red X in CTM task list for the day

  • Once a task is created, you can change the due date when task is accomplished for the month (updating every 30/60 days)

  • Your Care Team cohorts (and your supervisors) are looking to tasks to understand what you are doing/what your plan is for this member

  • Check your task list daily. You should have no overdue tasks when you leave for the day. You can change the due dates if your schedule has changed in order to organize your week.
