Engagement Dashboard-Full Leader Guide

This article is to know how to use engagement dashboard for daily tasks

With this update, all SPHM Health market team members with a primary care team role, along with their reporting managers, will have access to the new dashboard.

The primary purpose of this dashboard is to offer real-time visibility into the team's pending tasks for the week while also providing an overview of each primary care team member's progress.

Below are few important points to consider

  • Both the care team and the managers can look at the Hierarchy chart and view the dashboards of their team or other market team. But when they click on any value in the dashboard the members they can see on the member list would be only of the members they can access to

There is a switch for the manager dashboard to view their own case load tasks or high level pending tasks of their team members.

Drill Down:

Clicking on this the managers can view the detailed breakdown of each of the buckets within each category.

This dashboard is to track and manage engagement efforts at different organizational levels (CE, Manager, Reporting Manager) by providing real time visibility for the following segments:

  1. Hospital Notifications

After mMR receives a hospital notification or when a hospital visit is added to the member profile under hospital visits tab. The primary care team is responsible for outreach until a case note with Hospitalization/ED intervention and successful status is achieved.

This category provides the count of total members pending to outreach & that are due to outreach today.

Outreach Today

1st attempt – should be within 24 hours.

Follow up attempt – Every 2 business days

Ex:

Notification comes on 2/24

First attempt before end of 2/25

Second attempt before end of 2/27

Third attempt before end of 3/3

Reach out till the end of 30 days

Member will be removed from the list after 30 days/when there is a Successful and substantive engagement case note with Intervention selected as "Hospitalization/ED intervention

  1. Missed Engagements

Manager level => Sees only overdue missed engagements from the team.

CE level => Sees both open and overdue missed engagements.

  • Open and Overdue display the counts of engagement requests assigned to the CE for all the members accessible to them. Clicking on counts would take you to the engagement requests page and apply the filters to show those engagement requests.

  • If the logged in CE is eligible to see the new Missed Engagements section then we will show the counts related to the Missed Engagement.

  • If the CE is not eligible we will show the title as Engagement Requests and show the counts related to old ERs.

  1. Weekly Engagement Progress

Total Members:

All the active members who have passed the intake workflow

Not Engaged:

Members who the CE is part of primary care team and has no case notes or any follow up appointments scheduled for the week

With Upcoming Appointments this Week

Members with upcoming appointments for this week

Unsuccessful Engagement

Members with no Successful and Substantive engagement case note in that week but have some(less than 3) engagements by role of the logged in CE (ie. CE has attempted to engage, <3 times)

4. Intake Progress

For the intake progress we consider the members with “Intake” workflow.

Without intake

Of the total members in intake we categorize the count of members who have no intake appointment scheduled in future.

In this bucket there are different classifications of how many days the members are in the intake status and without intake.

14+ days => Days range 14-29 days

30+ days => Days range 30-58 days

59+ days => More than 58 days in intake status

With Scheduled Appointments:

This bucket displays the count of members who have an intake appointment scheduled out of the total members in the intake workflow

5. Disengaged Members

  • For this report we will consider all the members who have crossed the intake workflow and in active status.

  • Members who are disengaged for more than 7 calendar days are considered under “All Disengaged”

  • If any member is disengaged for 90 days we will show them under the Ready for Discharge section. This is a subset of All Disengaged members.

Member Filters:

Added new option “Dashboard” under metrics. These filer options get auto applied when navigated from the dashboard.
